Trevor Michael Dworshak, my precious angel, passed away on May 7, 2013 at Primary Children's Hospital in Salt Lake City, Utah. He was born January 28, 2013 in Ogden, Utah. He is the son of Heather Lea Dworshak and the baby brother of Braxton Allen Bentley, Jessalyn Marie Bentley, Shilo Braxton Weaver, and Kolton James Dworshak; he is the grandson of Laura Marie Dworshak. Trevor was preceded in death by two of his older siblings, Braxton & Jessalyn. There will be graveside service on Monday, May 13, 2013 at 1:00 p.m. at the Elwood Cemetery, with a viewing at Rogers & Taylor Funeral Home Starting at 12:15 p.m.
